Sussex Community NHS Foundation Trust awarded for best practice initiatives by Community Hospitals Association

Sussex Community NHS Foundation Trust (SCFT) is celebrating success in Horizon Unit at Horsham Hospital and on Piper Ward at Crawley Hospital after winning three awards and a commendation for Innovation and Best Practice by the Community Hospitals Association (CHA).

Team members Dawn Fincham, Katherine Taylor, Cathy Farey and Irene Owen and Samantha Holdham were presented with the National Innovation and Best Practice Awards for three aspects of best practice including the Hospital to Home Clinic, Daily Safety Huddles and Treatment Escalation Plans and were highly commended for their work surrounding Dusk Cafe, an initiative implemented at Horsham Hospital last year.

The Hospital to Home Clinic was set up in response to a need to improve patients’ transition from hospital to home, increase support for patients and enable them to remain independent at home. A clinic was set up where patients and carers could meet with agencies, including from the voluntary sector to discuss support available on discharge.

Dr Helen Tucker, President of the CHA presented the award alongside Richard Hallett MBE and applauded the way in which community hospitals are emerging to fit in with a changing NHS while reflecting the needs of local communities.

Community healthcare featured heavily in the NHS Long term plan in January which describes a focus on community and primary care services to meet the growing demands of an ageing population.

Evelyn Prodger, Inpatient Services General Manager at SCFT said:                         

“I’m really proud of the way the teams continue to meet the needs of patients and have the ability to take an idea and turn it into something that really does make a difference.”

SCFT Chair, Peter Horn said:

“Today we have heard about three innovations that have improved services for patients. Innovations that have been deservedly recognised by CHA and have started at ward level, not imposed from the top but created by the hard work and ingenuity of staff. A big ‘Thank you’ to everyone involved in these initiatives.”
